I was able to actually find similar behaviors in a few spots - it seems the character can sometimes push past objects into small spaces that they then cannot push OUT of. It may be more of a character controller issue than a level issue.

Yes indeed, this is not the first time that we saw this, and we haven't found a generic way to avoid it yet. So far we were just avoiding it with careful level design, which is not a final solution. #1395
