• add tab-aware docs tables list, docs images list, docs headings list, and docs paragraphs list
• provide structured JSON envelopes and headerless escaped TSV under --plain
• preserve stable source ordinals through filters and recurse through table cells and table-of-contents content
• emit image object IDs, document anchors, positioning, alt text, and dimensions in document order
